The Condition Management Programme (CMP) is a work focused programme which can provide a wide range of interventions and support to your patients who have a physical and/or mental health condition which affects their ability to work.
The team is made up of Occupational Therapists, Physiotherapists and Mental Health Nurses and each patient receives a full bio-psycho social assessment of needs and may require interventions from one or more Clinicians as part of their participation in the programme.
CMP is a well-established, clinically proven, free programme which can offer up to a maximum of 12 sessions of 1:1 and or group intervention.

We are proud to support our local community and are highly experienced at providing eyecare at home to those who are unable to leave unaccompanied due to a physical or mental illness or disabiltity. We are passionate about improving the eye health of patients and providing advanced eyecare services on behalf of the NHS in the community, enabling an improvement in the quality of life of our service users, all while in the comfort of their own home.
Who qualifies for a home eye test?
To be eligible for a home visit, someone might:
- Have a condition that stops them from leaving home unaccompanied due to poor health
- Be living with a diagnosed mental health condition that prevents them from leaving home without the assistance of another person
- Be housebound or bedbound due to a physical disability
- Our team will speak to the patient, or their caregiver, to determine eligibility for a home visit. The team will assess if they also qualify for an NHS-funded eye test.

The Direct Assessment Unit (DAU) at Antrim Area Hospital provides Same Day Acute Care, meaning patients have access to rapid assessment and treatment the same day as referral. NI Pears
NI PEARS is a regional service provided by community optometrists for patients who develop an acute (new and sudden or very recent onset) eye problem e.g. sudden red, painful or sticky eye, sudden flashes & floaters, sudden reduction in vision, something in the eye that you cannot remove. Contact a local optometrist for investigation and management of your acute eye problems. Patients with non-acute eye problems, including non-acute headaches and previously diagnosed chronic eye conditions, should not attend a PEARs clinic but contact the GP via triage.
If the patient is not eligible for NIPEARS or a GOS sight test there may be a charge.
The Phone First service is designed for patients who are feeling unwell and considering travelling to an ED with an injury or illness which requires urgent treatment but is not immediately life threatening. It is important to note that Emergency Departments will always be a safe place for patients, and if they attend an ED without ringing first, they will not be turned away. If their condition is not life threatening, they may have to wait longer or be signposted to another service.

Pharmacy First Services are offered in the majority of community pharmacies across NI. These services provide an opportunity for suitable patients to attend community pharmacies for advice and treatment for a range of conditions.

Emergency Hormonal Contraception: This service provides Sexual Health Advice including risks of STIs and how to get tested, emergency contraception and if clinically appropriate 3 months’ supply of desogestrel bridging contraception, safe-guarding, and signposting.

UTI service for female patients aged 16 to 64 years: This service facilitates the assessment and treatment of women aged 16-64 years presenting with symptoms of uncomplicated LUTI. Further details are available at Pharmacy First, click the button below:

You can also avail of over the counter treatment and advice for:
- coughs, colds, eye conditions, stomach upsets, skin conditions, allergies, aches and pains
- common drugs, vitamins and minor first-aid
- healthy eating and living, including giving up smoking
- blood pressure and diabetes monitoring and needle exchange, truss fittings, stoma products and incontinence supplies
- women’s health, including treatment for thrush, emergency contraception and pregnancy testing
- children’s health, including nappy rash, teething, coughs and colds, threadworms, head lice, warts and verrucas
- all prescribed and over-the-counter medicines
Travel Vaccinations
The Practice no longer offers a Private Travel Vaccination Service. There are however, various clinics in Northern Ireland offering this service, the most local at Carrick Health Centre Pharmacy, Taylors Avenue, (contact information can be found on line for other clinics in Belfast/Bangor).

- Allow plenty of time for a pre-travel consultation at least 6-8 weeks before you travel.
- A dental check-up before you travel may prevent problems while you are away.
- Take out adequate insurance for your destination and activities. A European Health Insurance Card (EHIC) entitles you to free or reduced rate medical care in most EU countries.
- Pack a first aid kit (a sterile kit of emergency equipment may be a good idea if you are going somewhere remote).
- Find out about the place you are travelling, the Foreign and Commonwealth Office website www.FCO.gov.uk contains information and up to date advice on travelling abroad, including information about risks in specific countries.

Haemochromatosis and Blood Donation.
National blood donation services would like you to encourage eligible patients & carriers with haemochromatosis to come and donate blood.
Carriers of a single HFE gene mutation don’t require referral to donate blood, however, those affected require a formal referral to be completed with a recommendation on the donation frequency. By donating blood you can support the NIBTS against the increasing blood demands
Please note that blood donation can be booked through the online booking system, where donors can pick and choose dates and times in advance, but on occasions donors can be discouraged to donate, based on our blood stock level or the requirement of their blood group.
