Confidentiality and Medical Records

The practice complies with data protection and access to medical records legislation. Identifiable information about you will be shared with others in the following circumstances:

  • To provide further medical treatment for you e.g. from district nurses and hospital services.
  • To help you get other services e.g. from the social work department. This requires your consent.
  • When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care.

If you do not wish anonymous information about you to be used in such a way, please let us know.

Zero Tolerance Policy

The NHS operate a zero tolerance policy with regard to violence and abuse and the practice has the right to remove violent patients from the list with immediate effect in order to safeguard practice staff, patients and other persons.

We aim to treat our patients in a courteous and respectful manner at all times and in turn expect our patients to behave in a similar way. We take seriously any threatening, abusive or violent behaviour against any member of staff, administrative or clinical.

If you are unhappy with the standard of care or treatment you have received we would encourage you to tell the person you’re dealing with at the time so that, if possible, your problem can be sorted out straight away.

Did Not Attend (DNA) Policy

Practice Policy relating to Non-Attendance at pre-booked appointments.

This Practice invests a significant amount of time trying to create and maintain an appointments rota which will adequately manage the levels of supply and demand. All factors including doctors training, annual leave and known times of ‘higher demand’ (e.g. after Bank Holidays and during the winter months) are taken into account.

Unfortunately, there has been a growing problem with the number of wasted appointments which are caused when a patient does not attend (DNA) a pre-booked appointment with a GP, Practice Nurse, or visiting specialist clinician. Whilst the Practice is committed to providing appointments to meet the needs of patients it cannot replace appointment time lost when a patient fails to show up, or when they cancel their booking so late that the slot cannot be re-used.

IMPORTANT PATIENT NOTICE:

Due to our current clinical system provider ceasing to operate, Scotch Quarter Practice will move to a new computer system from Friday 5th December 2025. This will impact on services we can provide between 5th December  to 12th December.

  • Emergency Only Service Friday 5th December to Wednesday 10th December
  • Limited Appointments Thursday 11th December to Friday 12th December

If you have a prescription due between these dates you MUST order no later than 3rd December 2025. The practice will have no facility to process prescriptions until after the installation of our new system. If you are an online user you will receive an email notification from [email protected] on how to register for the new online system once fully operational – (this is not spam). You can continue to use online services until 28th November 2025. We have currently ceased new applications for online services.
